The Licensed Practical Nurse LPN is responsible to provide daily nursing care, order medications, provide updates to the RN and Nurse Practitioners, train and orient AMAPs, schedule appointments, participate in Center activities and complete required documentation. Multiple shifts availableA.A.S. Degree or High School plus Specialized Trade Certificate. Valid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate. Basic Computer Skills Windows, Outlook, Adobe Acrobat, Word, Excel. This position has regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under OPWDD Office for People with Developmental DisabilitiesTravel and transport is required. Must have a valid NYS Drivers License and require reliable transportation.An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.Promote the individuals recovery by implementing the treatment or rehabilitation plan. Provide support and consultation and initiate peer support among program participants. Oversight and participation in medication administration, tube feeding, diabetic injections and ordering of medication. Promote program participant input in the development of program activities. Oversee the safety and well being of the program participants.Oversee and upkeep the maintenance of equipment and supplies.Participate in activities as part of the treatment team that may include meetings, appointments, trainings, and committees.Maintain documentation as required. Adhere to mandatory reporting requirements and HIPAA laws.
