Salary/Grade:$58,000 per year; Grade 10, Exempt

Responsibilities
Description of Position: The Lead Case Manager provides guidance in all aspects of case management that include technical assistance in Client Track and facilitation of the Clinical Team and SELF group. The Lead Case Manager is responsible for client assessment, service plan development, regular contact with the families to assess progress and needs, provision of guidance and counseling to clients as needed, making referrals to social services not provided at the shelter, preparation of applications for housing, documentation of resident progress and of services, ensuring completion of required forms, staff consultation, and completion of all client track data input for residents. The Lead Case Manager is a part of the Leadership Team and supports communication and programming is consistent across both sites.
Qualifications
Requirements: Master’s degree in social work. 2+ years case management experience. Training in trauma-informed care (Sanctuary Model Training) preferred. Must have excellent verbal and written communication skills. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check. Must have an understanding of, and appreciation for, the Mission of The Salvation Army and the Sanctuary Model.
