  Home Care Case Manager - Salt Lake City - Full TimeAt Visiting Angels, we are committed to providing compassionate home care services to seniors in Salt Lake County. Meeting our clients' needs is essential to ensuring that they can continue to age with dignity in their homes.

  The Case Manager will work closely with caregivers, clients, and family members to deliver individualized care for our clients. You will assess our client's needs, develop customized care plans, and work with our caregiving team to provide first-rate ongoing service. This is a great fit for you if you're interested in serving the senior population, can connect easily with new people, are confident in building relationships, and would enjoy representing Visiting Angels to the larger community.

  (We are looking for LOCAL Salt Lake County candidates with 3+ years of previous experience in case management. This is NOT an entry level position).

The Case Manager will report to our Agency Manager, work with our team in person at headquarters in Millcreek, Utah, and conduct in-home assessments throughout Salt Lake County.

  Ideal applicants will have worked with seniors (personally and/or professionally) and have at least 3 years of experience working in Case Management in settings such as Home Health agencies, Hospice, Geriatrics, Social Work, or Care Coordination. Clinical skills for coaching caregivers and providing direct care training. Current CNA or willing to obtain a CNA. Soft sales experience to close the deal to enroll new clients.
