FTE: 1.000000 Shift: Shift 1 Job Summary: The VAD/Heart TransplantCoordinator will be responsible for the coordination of care for allpatients requiring implantation of a cardiac assist device. Theseinclude patients accepted for, and awaiting heart transplant, or thosewith Advanced Heart Failure in which the device is implanted as apermanent treatment option. The VAD/Heart Transplant Coordinator willalso be responsible for the coordination of care for all patients whoare pre- or post- heart transplant. The VAD/Heart Transplant Coordinatorwill work as a member of the Advanced Heart Failure Team, and will bethe primary liaison with the transplant cardiologists and cardiactransplant surgeons, other physicians, and health care staff throughoutthe hospital. A major component of the position is patient and familyeducation, daily rounds, discharge planning, and on-call coverage. TheVAD/Heart Transplant Coordinator is also responsible for outreachactivity to promote the programs, frequent communication with referringphysicians, and professional education. The VAD/Heart TransplantCoordinator functions within the parameters outlined in the LVADpractice guidelines. The Job Standards portion of this job descriptionhas incorporated the 17 standards adapted from the ANA Nursing: Scope &Standards of Practice, 2nd Edition (2010). Other duties as assigned.EXPERIENCE DESCRIPTION: Minimum of 3 years of acute care or experienceis required. Experience relevant to acute care, cardiology, heartfailure, VAD care is preferred. EDUCATION DESCRIPTION: Associate\'sdegree in Nursing with a minimum of 10 years of relevant experience isrequired or Bachelor\'s degree in Nursing with a minimum of 3 years ofrelevant experience is required. Bachelor\'s Degree in Nursing ispreferred.SPECIAL SKILLS DESCRIPTION: Analytical skills necessary inorder to prepare and implement effective nursing care and developsolutions to problems concerning patient\'s mental or physicalwell-being. Ability to concentrate and pay close attention to detail forup to eighty percent of work time while: planning and performingprofessional nursing care, resolving patient care problems and dealingwith patients, their families, and other professionals involved in thepatients care, often during stressful...
