Summary:
Helps patients and family members navigate successfully through the health care system, by removing barriers to care, including providing access to resources, support and enhancing communication with the health care providers. Under the direction of the area manager, schedules appointments, monitors for care gaps, and takes action to address these gaps. Focuses on the care navigation of patients within the CT low dose lung screening program, within the breast screening program, and/or patients identified with incidental findings. Effectively and professionally communicates with radiologists, and other care providers to address barriers to care. Completes necessary documentation and tracking within appropriate systems to meet regulatory and accreditation requirements
Qualifications:
Required:High School diploma or passage of a high school equivalency exam.Three years of scheduling or related function within a healthcare environment.Strong computer skills including Microsoft Office software products.Demonstrated effective communication and interpersonal skills.Ability to work in a team environment.Culturally sensitive and comfortable working with a diverse population.Organized and detail oriented.Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds..
Preferred:5 years of medical health care experience.Previous medical terminology educationPrevious experience with medical insurance products.Bilingual or multilingual skills.
Physical Demands:May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating.May need to remain still for extended periods, including sitting and standing.Ability to communicate in face-to-face, phone, email, and other communications.Ability to read job related documents.Ability to use computer.
