　　RN Coordinator role consists of the planning and coordination of new referrals. The position will also provide phone triage of existing patients. The majority of the responsibilities will be carried out onsite in office. May provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and/or caregiver, the physician and the payor. These services are provided in the states of Oregon and/or southwest Washington.

　　Required Qualifications:

　　Education to meet certification, license or registration requirement

　　Oregon Registered Nurse License upon hire or

　　Washington Registered Nurse License upon hire or

　　National Registered Nurse Compact License upon hire

　　National Provider BLS - American Heart Association upon hire

　　Unencumbered Registered Nurse License in state(s) of hire, and wherever care is delivered upon hire. If RN holds a multi-state license through the Nurse Licensure Compact (NLC), the RN must adhere to the requirements of licensure in the new state(s) of hire
