Posting Summary
Rutgers, The Statue University of New Jersey, is seeking a Resource Coordinator for NJMS-Medicine-Infectious Disease. The Resource Coordinator provides support to the program to ensure the success of the medical case management program.Among the key duties of this position are the following:
  • Responsible for conducting chart audits, quality improvement and quality assurance activities, assisting with training of staff, and helping to identify ways to improve programmatic efficiencies and outcomes.
  • Relays messages to clinicians and follows-up as indicated by client, MCM, and supervisor.
  • Provides client-level support to clients living with HIV/AIDS with medical case management (MCM) services as defined by the Health Resource Services Administration (HRSA).
  • Performs a biopsychosocial assessment and develops a care plan to address the client's needs initially and at a minimum every six months
  • Key MCM activities include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) provision of services required to implement the plan; (4) monitoring to assess the efficacy of the plan; and (5) periodic re-evaluation and adaptation of the plan as necessary over the life of the client.
  • Monitors, inspects, and proposes measures to improve medical case management performance. Provides ongoing support to staff documentation requirements.

Minimum Education and Experience
  • Bachelor's Degree in a Health, Public Health, Social Work, or other relevant field required, plus three (3) years of relevant experience.
  • Five years of case management experience or NJDHSS HIV counselor certification preferred. Computer literacy with competence in Microsoft Office components required.
  • Equivalent education, experience, and/or training may be substituted for the degree requirements.

Physical Demands and Work Environment
PHYSICAL DEMANDS:
Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently. Extensive reading and extended sitting, ability to move from unit to unit. Position requires standing and walking over 2/3 of the time. Talking, hearing & sight are essential; requires clear close & distance vision, peripheral & color vision, depth perception and ability to adjust focus. Position requires lifting to 10 lbs.; on occasion more based upon client demands. Assist with transportation services in the clinic.WORK ENVIRONMENT:
Occasional (less than 1/3 of time) exposure to bloodborne pathogens that requires use of personal protective equipment.
