JOB REQUIREMENTS: Overview Provides expert support to the community andpost acute care facilities specializing in hospice outreach, educationand hospice admission. Responsible for coordinating and completinghospice admissions, coordinates communications between nursing homestaff, physicians and hospice staff. Serves as a resource to facilitystaff; physicians; patients and family caregivers to ensure of apatient-centered transition to hospice. Responsible for ensuringcompliance of Home Health Certified Nursing Aides with IDPH rules andregulations. Responsibilities Collaborates with referral source,physician, Central Referral Source Center, and case manager to identifyand complete appropriate hospice admissions. Acts as resource to allfacilities in promoting appropriate level of hospice care for qualifyingpatients. Serves as the point person responsible for conducting theinitial hospice presentation to establish realistic expectations ofprogram and services. Coordinates acceptance into hospice in LTCfacilities. Performs initial assessment through direct patient contactand patient/caregiver interview; assessing the patient\'s needs; thecaregivers ability to provide care and the safety of the homeenvironment. Determines if patient meets Medicare/Medicaid eligibilityrequirements. Knows and is up to date on eligibility requirements forhospice and can proficiently communicate these findings to physicians,discharge planning, other healthcare professionals and patients andfamilies. Completes all paperwork in a correct and timely manner. Isresponsible for gathering and communicating information and maintainingon-going communication with VNA\'s referral resource center and hospicedepartment. Establishes and maintains a strong and cooperative workingrelationship with the long term care discharge planner and otherappropriate healthcare staff at the facility. Works in collaborationwith hospice staff and facility staff to enhance effective communicationand nurture relationships. Serves on the Hospice Interdisciplinary Team.Demonstrates a broad knowledge of the rules and regulations governinghospice. Assists with hospice education within the nursing homesproviding in-services and training to meet regulatory requirements. Mayprovide skilled hospice care to patients in hospital, facilities and athome when needed. Will maintain skill set of hospice staff nursecomplying with competencies per hospice nurse position. Participates indeveloping and implementing hospice policies guidelines and proceduresand quality programs and initiatives as directed. Accepts assignedsystem and department committee memberships in promotion of VNA andHospice mission and quality.
