Summary: Integrated Care Consultant functions as a part of the primarytreatment team. The Integrated Care Consultant provides assessments,identifies patient who would benefit from integrated care interventions,treatment plans to address and identify goals, evaluates crisissituations and intervenes appropriately, educates patients on preventionand treatment options, collaborates with primary care team to assistwith integrated care of patients.
Job Responsibilities:
Provide assessment and diagnosis of behavioral health and chronic carepatients
Plans treatment for patients to assist with achieving identified chroniccare and behavioral health goals
Identifies patients from patient population who would benefit frombehavioral interventions.
Provide behavioral interventions
Consults with medical providers, medical assistants, and nurses toassist with patient center care
Educate patients, their families, and HHC staff on prevention andtreatment of chronic conditions and behavioral health conditions
Create process for follow up care for patients to assist withmaintaining and managing their goals
Identify, evaluate, and intervene with crisis situations
Attend clinic and other meetings as assigned
Maintains clinical record in electronic health record and other requiredpaperwork in timely manner
Complete SBIRT interventions and refer patients to appropriate servicesas necessary
Collaborate with HIV/AIDS outreach staff to assess, consult, andintervene with patients who are completing an HIV/STD test
Other duties as assigned.
Qualifications:
Masters degree in psychology or Social Work, licensed in the state ofKansas, is required.
Licensed Specialist Clinical Social Worker (LSCSW) preferred.
Minimum of two years experience in behavioral health services preferred
Experience with integrated care preferred
A bilingual individual is preferred, but not required.

Working Conditions: Works in a well-lit, climate-controlled environment.The noise level in the work environment is usually high. Potentialexposure to infectious disease is possible.
Physical Strength: The employee must occasionally lift and/or move up to30 pounds.
Mathematical Ability: Ability to add and subtract two-digit numbers andto multiply and divide with 10\'s and 100\'s.
