About Community Care Associate - FT Days - Park Rapids Clinic

  Job Description:

  Seeking a Community Care Associate to serve at our Park Rapids Clinic, connecting individuals and families to health-related community resources, mental health supports, and substance use supports. This employee works as an integrated member of the clinic team, 40 hours per week, Monday through Friday. No weekend, holiday, or on-call rotations are required. Requires an individual who has interpersonal, organizational, problem solving, and critical thinking skills. The right candidate must exhibit professionalism, positivity, and respect towards differences in others. Functions as a member of the primary care team (patient and family, primary care physician and advanced practitioners, behavioral health providers, and other clinic staff) and a member of a community network serving patients with complex challenges. Partners with patients and their families to identify needs and create a plan to address them, with emphasis on behavioral health and social determinants of health. Coordinates services to increase access to medical, behavioral health, and community services. To improve patient overall wellbeing, the community care associate develops a trusted relationship with the patient and their family, completing an assessment, making referrals, and following-up.

  Licensure/Certification Qualifications:

  Community Health Worker (CHW) certificate preferred.

  $17.91 - $26.87 / hour

  The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package. The offer will be determined by a number of factors including the selected candidate’s experience, knowledge, skills, abilities, FTE, as well as internal equity among our team.
