Description Lutheran Social Services of Wisconsinand Upper Michigan is currently seeking an excited Care Coordinator towork with Refugees! This role with work with Ukrainian HumanitarianParole clients providing intake and assessment in a 90 day program,connecting clients to needed services and benefits. This is a full timebenefit eligible role. Under direction, performs work involving theassessment, service planning, and implementation of Core Services andon-going case management for assigned individuals. Performs other dutiesas required.
