Senior Facilitator
Family Access & Connection Team (FACT)
Rochester, NY On-Site
FULL-TIME Nonexempt : 40 hours/week
Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033539) seeks a talentedSenior FACT Facilitatorto contribute their expertise in providing essential business services to our provider organizations and customers. TheSenior FACT Facilitatorwill carry a caseload of youth/families presenting with high need and high risk. TheSenior FACT Facilitatorwill utilize his/her knowledge and experience to assist the FACT Supervisory Team in providing consistent training to newly hired staff and coordinate ongoing training needs of FACT staff. TheSenior FACT Facilitatorwill support and be available for consultation to the entire FACT staff to best recognize the strengths and meet the needs of the youth and families FACT serves.
CCSI is partnering withMonroe County Department of Human Services (DHS)to hireaSenior FACT Facilitator.This position is employed by CCSI and working on behalf ofMonroe County DHS.
In this role, you will
Conduct strength-based and cross system assessments on a reduced specialized caseload, including Aftercare cases.
Convene and facilitate Family Meetings to create an individualized plan of care that is based on a family's strengths and needs, maximizing the use of informal supports.
Build effective linkages and relationships with informal and community supports, child and family serving systems, schools, and other formal supports.
Complete all systems requirements of MCDHS, MC Probation and MCOMH.
In conjunction with the Leadership Team provide thorough and comprehensive training for all newly hired Facilitators regarding FACT policies, procedures, and protocols.
Provide opportunities for newly hired Facilitators to attend/observe Family Meetings, School Meetings, etc.
Act as liaison to both internal and external partner agencies
Provide support as needed to grant-funded or special projects
Facilitate case consultation and professional development opportunities for Facilitators.
Work flexible hours including evenings and provide supervisory coverage in the absence of other Supervisory staff, including coverage of the Information line.
Develop weekly evening coverage schedule.
Provide coordination and guidance in facilitating additional FACT programming such as the Boys Council and Girl’s Circle groups.
Other duties as assigned.
Requirements
Master’s degree in a human service related field preferred (May substitute a Bachelor’s in a human services field if at least 2 years of Supervisory/Leadership experience) or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) ).
Experience in mental health, child welfare, juvenile justice, and/or school systems preferred.
Additional Requirements
Satisfactory completion of all required background screenings.
Valid Class D New York State Driver License and access to motor vehicle to meet transportation requirements of position.
Ability to work flexible hours including evenings.
