Registered nurse shall have successfully completed a degree in Nursing from aNational League of Nursing or from an American Academy College of Nursingaccredited college or university. BSN preferred
Registered nurses shall hold a national or state nursing license that is validand current with no limitations, stipulations or pending adverse actions.License must be unencumbered and remain in effect during contract employment.
Minimum of three (3) years experience as a Registered NurseMinimum 2 years experience full-time experience in care coordination,discharge planning or case management in a medical setting within the lastfour years.
Registered Nurses shall have a current BLS through the American HeartAssociationCertified/certification eligible in relevant specialty, such as CertifiedManaged Care Nurse through the American Board of Managed Care Nurses orCertified Informatics Nursing, Ambulatory Care Nursing, Medical-SurgicalNursing or Nursing Case Management through the American Nurses CredentialingCePosition Qualifications:Education: Registered Nurses (RNs) shall have successfully completed aBachelor of Science Degree in Nursing (BSN) from a National League ofNursing (NLN) or Commission on Collegiate Nursing Education (CCNE)accredited college or university.Active state or compact RN license to work in the assigned areasExperience: RNs must have at least two (2) years full-time experiencein Care Coordination, discharge planning or case management in medicalsetting within the last four years.Licensure/Registration: RNs must possess a current, active, full andunrestricted license in any U.S. State or jurisdiction, be able to practiceusing a nurse compact state license or Enhanced Nurse Licensure Compact(eNLC) or have a license from any United States jurisdiction.Ability to handle confidential patient information.Have knowledge of computer operations and proficiency in the use of basic wordprocessing, data entry, and automated recordPosition Duties:The HCW will be providing care as the Special Needs Coordinator (SNC)under the Exceptional Family Member Program - Medical (EFMP-M) to includebut are not limited to the following:* Exhibit 5 Core Duties (Medial Social Worker or Registered Nurse)* The SNC oversees and manages the installation EFMP-M IAW DoD and AF policyand any subsequent implementing guidance.* Uses clinical assessment skills and maintains procedures to identifysponsors whose family members have special medical and educational needs in atimely manner. The SNC uses a multi-disciplinary and collaborative approachwith other key service providers, such as installation youth and childcarefacilities, and officer and enlisted spouses' groups, to ensureeffective outreach and identification of special needs. Ensures family membersof the AF personnel or family of the AF member are appropriately "Q coded"(EFMP-enrolled).* Ensures all active-duty sponsors known to the Air Force Personnel Center,and local Military Personnel Section (MPS) and/or Commander SupportStaff (CSS) where applicable, having family members with special needsare identified in AF medical special needs data management systems. Providesoversight for base-level data entry in AF-provided data manage ent systemsused in the management of EFMP-M.* Provides training to installation personnel and medical staff; andconsultation as needed to support the implementation of EFMP-M base-wide.* Integrally involved in the Family Member Relocation Clearance (FMRC)process. Ensures all FMRC requirements are implemented by all EFMP-M staff.Coordinates the enrollment process and travel screening for all active-dutyfamily members' and travel screening upon request to the families of DoDcivilian sponsored assignments going to an overseas base.* Provides oversight to ensure every AF sponsor with one or more familymembers with special needs assigned to the installation has a SN filemaintained at the Medical Treatment Facility (MTF).
