　　As a NP at Horizon, you will…

　　Provide history and physical exams for new admissions and transferred patients, evaluation and treatment for acute and chronic medical conditions, and provider Medication Assisted Therapy assessments for individuals admitted to mental health, chemical dependency, and rehabilitation and recovery programs.

　　Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.

　　Prescribe appropriate medication to treat or ameliorate symptoms.

　　Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.

　　Review and sign all treatment plans for chemical dependency programs within 7 days of the multi-disciplinary team approval --- as needed.

　　Document all services provided in the case record. Must be literate with electronic health records.

　　MD or DO, NP NY state license and board certified (minimally board eligible)

　　1 year preferred, but not required in mental health or substance abuse.

　　Strong preference for individuals well versed in addiction medicine.

　　Willing to travel to other clinics if needed.

　　Location:

　　This position would be located at two of our clinics: 3345 Southwestern Blvd, Orchard Park NY & 637 Davison Rd, Lockport NY! 3 days will be at the Orchard Park clinic, while one day will be at the Lockport clinic. You must be willing and able to travel between clinics if needed.

　　Hours:

　　This position is full-time (35 hours), from Monday-Friday.

　　Compensation:

　　Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $65.00 - $71.50 per hour. Starting pay may be impacted based on experience and education. This position includes a bonus earning opportunity, with an annual max potential of $2,400, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
