We are known for our commitment to high quality, customer focused outcomes. _Position Overview:_ Our Stratford, CT outpatient clinic is seeking a Physical Therapist specializing in Outpatient Orthopedics. The ideal candidate will have their OCS or have completed an Orthopedic Residency. The Physical Therapist (PT) independently evaluates, determines treatment strategies, delivers and/or delegates services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, with emphasis on physical dysfunction. This role encompasses evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, exercise and other procedures to maximize the patient's functional independence. Evaluation involves strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait and functional abilities in activities of daily living. *Responsibilities: * * Upon referral, the Physical Therapist (PT) performs thorough examination and evaluation of patient’s level of function, including strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function by assessing and monitoring vitals, endurance, gait and functional abilities in activities of daily living. The PT recognizes physiological, psychological, and age-related changes in the patient. * The Physical Therapist (PT) develops an individualized patient-centered treatment plan of care to promote optimum movement function and safety. * The Physical Therapist (PT) implements, delivers and delegates patient care by utilizing appropriate interventions and clinical decision making including but not limited to modalities such as heat, cold, light, water, sound, electricity, mobilization and therapeutic exercise. * The Physical Therapist (PT) helps to coordinate patient care with other care providers such as Rehab Therapists, Nurses, Social Workers and participates in patient transition planning and/or patient discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care. *Benefits:* As one of the largest rehabilitation providers in the state, HHCRN offers a team-oriented structure, a comprehensive continuing education program, an excellent benefits package along with Special Interest Groups and MedBridge subscription that assist with evidence based clinical knowledge, growth opportunities and support within the organization. * Flexible scheduling & competitive pay * 401K with company match * Clinical specialty opportunities We believe in high standards. Requirements * Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree * CT active or temporary license * CPR certification * Orthopedic Clinical Specialist (OCS) preferred * Completion of an Orthopedic Clinical Residency program a plus * Bilingual preferred We take great care of careers.
