　　Peer Recovery Specialist Job Description:Looking for a dynamic Peer Recovery Specialist to join our Community Support Program team. The Community Support Program is an innovative, integrated health care service delivery model that integrates behavioral health care with medical care for children and adults with serious mental health issues who also have serious chronic health conditions. Through an array of services the goal is to increase access to care, improve the experience of care, and to reduce unnecessary emergency and inpatient care while improving health outcomes.

　　Bring the peer perspective and your recovery experience to support and inspire others to achieve person-driven and self-defined achievable goals. Critical functions of the job include assisting in follow up with medical care, promoting involvement with wellness and health promotion activities, including self-help, and modeling successful recovery strategies.

　　Peer Recovery Specialist Schedule:

　　Monday-Friday8:30 a.m. to 5:00 p.m. An occasional evening or weekend day may be required.

　　Peer Recovery Specialist Minimum Requirements:Associates Degree or High School Diploma/equivalent plus two years relevant experience required.

　　This position requires certification as a Recovery Specialist/Recovery Coach through an approved agency Certification Provider.*

　　Reliable transportation and ability to transport clients in own car is a must. Mileage reimbursement is provided.
