  Summary:

  The RN Quality Manager works in collaboration with all members of the care team providing high quality, compassionate care consistent with the Mission and Values of Northern Light Health and SVH. The RN Quality Manager works closely with the VP of Physician Service, the Lead Primary Care Provider, and the Practice Directors for both Primary and Specialty Care, to provide clinical leadership and expertise as it relates to multiple quality performance measures and initiatives. They will be responsible for the collection, analysis, and synthesis of large amounts of data for both Primary and Specialty Care Services. The incumbent will be responsible for creating and monitoring progress of Performance Improvement Plans and will be prepared to present such plans to system and local leadership, as well as individual providers and teams. The RN Quality Manager will represent SVH on multiple System and local meetings, including but not limited to, the Primary Care Service Line, Beacon Population Health Conference and Beacon Working Group, Primary Care Standards and HMMUG (System Level) as well as SVH Performance Improvement Committee (PIC) and various local QI/QA committees. The RN Quality Manager must have an in-depth understanding of various quality programs, such as NCQA/PCMH, RHC Conditions of Participation, The Joint Commission and the MaineCare Primary Care Plus programs. They must also have a clear understanding of the quality components related to CMS, HEDIS and MaineCare reporting. The incumbent must be solution oriented and able to partner with other Departments and Service Lines to promote the best workflows for staff and outcomes for patients.

  Responsibilities:

  Provides clinical leadership and quality expertise for the Primary and Specialty Care Teams, and provides support to the Clinical Supervisors. 2. Obtains relevant clinical information and assesses for GAP in care, then creates and executes a performance improvement plan with a multidisciplinary care team approach to close such gaps. 3. Reviews and assists in other population health management functions 4. Supports and educates staff regarding the importance of conducting patient/family needs assessment to identify any barriers to care (i.e., learning needs, social determinants of health, need for additional services, etc.) 5. Participates in team huddles and team meetings to improve workflows, improve patient experience, and contribute to improving patient population outcomes 6. Maintains clinical/technical skills and knowledge through ongoing development and continuing education. 7. Assist the Practice Directors in ensuring all quality control audits are completed as scheduled 8. Provide staff education and auditing of Annual Wellness Visits as designated 9. Understands and properly utilizes the EHR and ancillary programs 10. Is able to work well independently 11. All other duties as assigned 12. Supervises direct reports and provides strategic direction to that work. Other Information:
