　　Job Summary and Qualifications

　　GENERAL SUMMARY OF DUTIES: Plans, organizes, coordinates, and evaluates the nutritional component of health care services for the organization. Provides direct patient care to adolescent, adult, and geriatric patients.

　　DUTIES INCLUDE BUT NOT LIMITED TO:  Develops and implements plans of care based on assessment of nutritional needs and available sources and correlates plan with other health care.  Evaluates nutritional care and provides follow-up continuity of care.  Instructs individuals and families in nutritional principles, diet, food selection, and economics while adapting teaching plans to individual lifestyle.  Provides consultation to and works with community groups.  Conducts or participates in in-service education and consultation with professional staff and supporting personnel of own and related organizations.  Perform other duties as required  Must read and understand and adhere to all Physician Services policies and procedures  Practice and adhere to the Code of Conduct philosophy and Mission and Value Statement

　　CERTIFICATION/LICENSE:  Registered must be ADA  License in accordance with state regulations

　　EXPERIENCE:  One to two years related experience or training. Age specific experience and/or special training and/or expertise is preferred but not required.
