QUALIFICATIONS AND EDUCATION REQUIREMENTS
Bachelors degree in Human Services field.Proficiency in spoken and written Spanish2 years of experience providing care coordination services to various populations
Effective communication (oral and written), Strong interpersonal, time management, and organizational skills.Responsible, self-motivated, ability to work independently and with a team, and capable of handling multiple taskWorking knowledge of computers including Microsoft Word.
DUTIES AND RESPONSIBILITIES
To provide short term case management services throughout Oswego County.Provide assistance with shelter, food, prescriptions and other basic needs as well as referring and linkage to other needed services.
Collaborate with other Catholic Charities program staff to help with understanding and rephrasing for Spanish speaking clients as needed. Collaborate with other Catholic Charities program staff as well as outside agencies to meet the needs of our consumersComplete all required documentation in timely and thorough mannerComplete tracking and stats as necessary Assist and perform other duties as assigned and as are applicable to the position.
ADDITIONAL EXPECTATIONS
Must have valid NYS Drivers License, an acceptable driving record and adequate insurance.Must comply with all Agency policies and procedures.Required to understand and abide by the laws, regulations, policies, and procedures that apply to the program or service that you provide.Required to report any known or suspected violation of laws, regulations, policies, or procedures related to programs, services, agency operations, or personnel of the agency.Required to maintain accurate documentation including, but not limited to, purchase orders, petty cash, expenditures, mileage records, and time sheetsAttend all trainings as necessary for job performance.NYS, OMIG, LENS, VIRTUS background check clearances
