About the Role:
The Community Rehabilitation and Treatment Case Manager is responsiblefor managing an assigned caseload of clients with major, severe,recurrent and long-term mental health disorders through coordination ofservices, provision of supportive counseling, evaluation and assessmentof needs. The CRT Case Manager shall maintain a close workingrelationship with all members of the treatment team in order to ensuredelivery to clients of comprehensive treatment in an efficient andexpedient manner.
Responsibilities:
- Participate and lead, as directed, in the screening of new servicerequests within the clinic through means of and agency approvedprocedures which ensures that all individuals are adequately andappropriately served according to their individual needs and inaccordance with existing agency policies and procedures.
- Accept newly assigned cases in which the diagnostic intake has beencompleted by a licensed provider. Complete case management assessmentand develop an individualized treatment plan with the client based onthat assessment for all clients on caseload. Provide on-going supportiveand/or case management functions in accordance with the problems, needs,and the strategies identified within the treatment plan in order to helpthe clients to achieve the stated goals and objectives.
- Provide on-going supportive and/or mandatory monthly case managementcontact, activity or communication in accordance with the problems,needs, and the strategies identified within the treatment plan in orderto help the clients to achieve the stated goals and objectives.
- Document all service contacts on a timely basis includingface-to-face interviews, collateral and networking contacts, and/orcorrespondence in addition to maintaining case records in accordancewith agency and regulatory standards and requirements.
Qualifications:
- Associate\'s degree in psychology, human services, social work orrelated field.
- Possess 1-2 years combination of mental health work experience andtraining to perform principle responsibilities.
- Knowledge with services, systems, and programs available in thecommunity including primary health care, support services, eligibilitycriteria and intake processes, generic community resources,
- Working knowledge of serious mental illness, mental disabilities, andsubstance use depending on the population served, including clinical anddevelopmental issues.
- Must have a valid driver\'s license, reliable transportation withpersonal automobile liability insurance coverage at or above the levelsmandated by the State of Vermont.
- *Successful completion of a criminal history background check afterhire.
