　　Performs activity based programs and visits in support of patient care plan.

　　Job Responsibilities and Requirements:

　　PRIMARY RESPONSIBILITIES

　　May conduct assessment of activity needs to meet individual treatment goals and objectives.

　　Facilitates and participates in activity-related projects and events.

　　Keeps treatment area clean, safe, and organized.

　　Maintains documentation of activity programs and participation.

　　Assists with setup and takedown of furniture and materials for activities.

　　May oversee volunteers.

　　Performs clerical tasks as needed.

　　Works in a constant state of alertness and safe manner.

　　Performs other duties as assigned.

　　EDUCATION

　　Occasional use of vision to judge distances and spatial relationships and to identify and distinguish colors.
