DescriptionSocial Services DirectorStatus: Full-TimeWage: $35 - $42/hr - DOELocation: Avamere Rehabilitation of Shoreline - 1250 NE 145th St, Shoreline, WA 98155Employee Perks:Tuition assistanceAccess up to 50% of your net earned income before paydayMentorship opportunitiesCareer DevelopmentEmployee assistance program featuring counseling services, financial coaching, free legal services, and moreGenerous employee referral programPaid time off/sick leave (rolls over annually)401K retirement plan with employer matchComprehensive benefits package with medical coverage, voluntary dental, vision, and life insurance, short-term disability, and AD&D coverageThe primary purpose of this position is to plan, organize, develop and direct the overall operation of our facility's Social Services Department to assure that the medically related emotional and social needs of the resident are met and maintained.ResponsibilitiesDevelop assessments and written plans of care to identify the needs of each resident.Ensure that all components of the care plan are being followed by reviewing MDS's and nurses' notes and attending meetings.Effectively communicate information concerning a patient's condition with other departments and government agencies.Participate in the discharge planning, providing information to residents and families of programs available.Seek out new methods and make recommendations on changes in policies and procedures based on current federal and state regulations.Assist in preparing and planning the department's annual budget, and maintain throughout the year.Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.QualificationsBachelor's Degree in Social Services, Master's Degree strongly preferred.2 - 5 years' experience in social services in a Skilled Nursing or similar health care setting.Registered with the Academy of Certified Social Workers or a member of the National Association of Social Workers.Knowledgeable of the rules, regulations and guidelines that govern nursing care facilities.Understanding of Medicare, Medicaid, Kaiser and private insurances.Excellent oral and written communication.Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.Please Note: Effective Oct. 18, 2021, all Avamere employees are required to be fully vaccinated against COVID-19 unless they have an approved medical or religious exception.
