  Under the supervision of the Registered Nurse and Program Director, and as a member of an inter-disciplinary treatment team, assumes case management responsibilities for assigned caseload of individuals who have mental illness and substance abuse disorder. Renders nursing care as assigned by registered nurse in accordance with recognized nursing techniques, procedures and established standards based on the scope of practical nursing.

  ESSENTIAL JOB FUNCTIONS:

  Provides symptom assessment, management, and supportive counseling. Administers and monitors medications, and documents activities in accordance with agency medication administration and monitoring protocol; keeps copies of all prescriptions written by providers in appropriate location.

  Assists in the formulation of a nursing diagnosis and treatment plan recommendations, including short-term and long-term goals as part of an inter-disciplinary team.

  Assesses resident/tenant’s health condition on a frequency set by the team; effectively communicates the person’s changing needs and plan of care among team members

  On a weekly, or as assigned, basis performs inventory of stock medical supplies and records and keeps an accurate count of controlled substances.

  Maintains medication records on a monthly, or as assigned, basis.

  Implements physician orders accurately, directly and indirectly.

  Reports a person’s needs, problems, and observations regarding their health to the registered nurse and/or physician.

  Observes assigned person’s behavior and specific responses to treatment and rehabilitation programs, support services and medication and reports observations orally and in writing, including computer entries, as instructed and in accordance with applicable audit and agency standards.

  Documents all significant behavioral responses in prescribed format; assists in crisis and emergency situations to protect the welfare of individuals, staff, and others.

  When necessary, participates in fire drills, performs headcounts, fire-watch and heat safety activities with other team members. Assists the clinical staff in accounting for residents/tenants. May assist in the filing of missing person reports on residents/tenants not accounted for in accordance with Institute for Community Living policy and procedure

  Maintains accurate and timely case records, including but not limited to: weekly progress notes, quarterly service plan reviews, assessments and medical records; makes daily log book entries, records all scheduled appointments in accordance with agency and regulatory guidelines.

  Provides crisis assessment and intervention.

  Provide assistance to residents/tenants in structuring and performing basic daily activities.

  Assists people in the development and support of social activities and relationships.

  Provides and/or arranges for emergency medical care.

  Conducts or assists other team members in providing informational and educational discussions and materials with people on topics such as: the effects of medication, coping skills, assertiveness training, sexuality, etc.

  May conduct basic first aid training for people receiving services.

  Provides information concerning health referrals such as: health resources in the community, etc.

  Participates in staff development activities and programs

  Ensures that medication room is kept in an orderly and clean condition.

  Maintains current first aid and CPR certification.

  Perform other related job duties as may be assigned.

  ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

  Basic knowledge of mental illness and serious emotional disturbances and substance use disorders.

  Basic knowledge of treatment, rehabilitation, and community support programs as they relate to residents/tenants, families, and staff.

  Basic knowledge of techniques for identifying and preventing potentially violent behavior, including crisis management techniques.

  Ability to work with people, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.

  Ability to communicate effectively with staff, people receiving services, families, and the public.

  Ability to complete written forms and reports in an accurate and timely manner, manually and by computer.

  Ability to be empathetic and supportive, instructing, demonstrating, modeling, and encouraging appropriate behavior and skills.

  Ability to secure and maintain certification for Standard First Aid and CPR.

  Ability to make accurate and timely observations, and decisions.

  QUALIFICATONS AND EXPERIENCE:

  Must possess valid NYS license as Licensed Practical Nurse and current NYS registration, plus at least one year of experience providing services to individuals with mental disabilities. Continuing employment is contingent on maintaining current valid NYS licensure.
