[Duties include direct care, advocating for, companionship and supervision of adults at one of Strong Solutions Assisted Living/Group Homes.
[We provide person-centered care, interaction, and assistance for residents with the following:[Activities of Daily Living, Person-centered personal assistance, Person-centered Interactions
[Strong Solutions Group Homes require daily housekeeping, paperwork, medication management and care logs, detailed notes and communication between the Administrator and Program Managers.

Detailed notesEnsures wholesome meal preparationEnsures appropriate cleanliness, maintenance, and safety of home.[Provides personal care and support services required for each support recipient, including, but not limited to, activities of daily living and community activities.]{century="" gothic",sans-serif"=""}Ensures each individual participates safely in a wide range of age-appropriateactivities.Ensures goals and objectives are being worked on as outlined in the SupportPlan.Coordinates, and provides if necessary, transportation to meet support recipient's schedules, includingpublictransportation and rides to work, appointments, andactivities.Monitors resident's health.Facilitates, coordinates, and/or participates in team meetings occurring during workdays, as required. Participates in staff training as needed.Accurately completes time sheets, daily log notes, service notes, goal tracking, and other documentation required while working in the home.[Schedule:Shared Live-In 3 days per week
