About Teacher-Early Intervention - Nurse Family Partnership - Part Time

　　Provides early intervention services to children.

　　Education/License

　　Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

　　Responsibilities:

　　Administers formal and informal assessments appropriately and interprets results.|

　　Recognizes and effectively addresses family's concerns. Ensures that reports and services reflect the family's concerns for their child.|

　　Makes appropriate referrals to medical and health providers. Coordinates with agencies involved with family. Collaborates and follows up as needed.|

　　Provides multi-disciplinary services and promotes family participation in incorporating services into daily routines.

　　The pay for this position ranges from $16.00-$24.35 per hour.
