　　Job Summary:

　　Provides evaluation, reassessment, and treatment of patients referred for home health speech therapy. Serves as a home health case manager on therapy only patients, taking responsibility for leading the care team, managing the total care plan and assuring adherence to regulatory and agency guidelines.

　　Job Functions:

　　Completes accurate evaluations, reassessments, and treatments of speech therapy patients. Develops patient POC and adapts it according to assessment

　　data and patients needs. Schedules work independently and according to patient clinical needs and agency policy. Documents care, progress, goals, contacts

　　and orders according to policy and regulation. Serves as a team member, communicating efficiently with patients/families/caregivers/MDs & 3rd party payors

　　Works autonomously without direct supervision in the field. Uses computerized technology for clinical care effectively after on the job training. Provides

　　appropriate patient/caregiver teaching. Leads teams in working toward patient goals for achievement in highest function possible, quality of life improvement

　　and community re-integration if possible. Leads team effectively including holding case conferences, Assures all team member's documentation is complete,

　　timely and meets standards. Assures patient/caregiver teaching, equipment needs, and home modifications are addressed relative to scope of service

　　provided. Competent in following guidelines for different payors and contracts and communicates effectively with them. Refers patients to appropriate

　　resources including Identification of need for other professional/agency intervention. Initiates discharge planning at onset of care based on patients needs and

　　modifies POC as appropriate. At discharge, assures coordination with any needed ongoing services and completion of discharge teaching.

　　Education: Graduate of ASHA accredited SLP program

　　Licensure: Kentucky licensed and Illinois license eligible

　　Certification(s) :BLS
