The Kalamazoo County Health & Community Services Department seeks a Licensed Master Social Worker to work in a newly created School-Based Health Center located within Loy Norrix High School in Kalamazoo, Michigan. This position, as well as an Advanced Practice Practitioner and a Medical Assistant, will comprise the staff providing physical and mental health services to the approximately 1800 Loy Norrix High School students.
Funding for this position comes from a long-term, stable revenue source from the Michigan Department of Health & Human Services (MDHHS). Additional funds have been provided to build the newly developed center, which will commence this spring.
Candidates for this position should be comfortable working in an autonomous environment and passionate about working with students to improve their lives and help provide a warm and welcoming medical center environment. GENERAL SUMMARY
The purpose of this job is to provide a range of services, including diagnostic assessment, therapy, and case management services for public health programs, including Maternal Child Health, Personal Health, and Area Agency on Aging, based on Division and role, as appropriate to the specific needs of the individual served. Collects data through case history and observational techniques, evaluates data to identify the root cause of an individual's needs, and determines proper therapeutic approach or referral to community services.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides therapeutic interventions, including counseling, therapy, and/or psychotherapy, to public health program participants within Maternal Child Health, School Based-Health Center in Personal Health, or Older Adults Divisions as appropriate to licensure and training.Prepares treatment plans and coordinates follow-up care; provides therapeutic crisis intervention for those with poor formal and informal support as appropriate.Evaluate the effectiveness of treatment used; make recommendations regarding creating and modifying a person-centered service plan, services, and service delivery, including coordinating essential services for clients.Collects data about clients using tools such as interviews, case history, psychological tests, and/or observational techniques; evaluates data to identify the root cause of an individual's needs and to determine the proper therapeutic approach or referral to community resources.Participates in, leads, and/or coordinates training sessions, community education, outreach initiatives, and workshops to enhance treatment skills and raise community awareness of public health concerns within Maternal Child Health, School Based-Health Center in Personal Health, or Older Adults Divisions.Consult with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refer clients to appropriate service agencies as required.Performs related work as required.
