Schedule: PRN - Minimum 4 shifts per month.
Shift : 8am-5pm (Monday-Friday)
Location: Hinsdale, IL Area
The role you’ll contribute:
Performs home visits as ordered by the physician. Prepares social histories. Assesses and evaluates the patient’s socioeconomic and emotional status, family relationships, and coping abilities and their relationships to the patient’s health status and wellness potential. Identifies needs, prepares, implements, and evaluates an individualized Plan of Care. Displays high degree of proficiency in the referral process with appropriate follow up. Provides high quality counseling and resource coordination. Promotes the team approach in the delivery of total patient care. Notifies the physician and other team members of necessary updates. Completes documentation according to Agency Policy and Regulatory guidelines.

· Evaluates patients as assigned.
· Assist the physician and other members of the home health team in understanding the significant social and emotional factors related to the state of health.
· Performs home assessment to evaluate potential for social needs.
· Conducts visits to evaluate patient needs and implement a plan of social service assistance maximizing patient care plan foals and providing for continuing management of needs within the home or seeking alternative arrangements.
· Counsel the patient and family members to develop attitudes and behaviors supportive of maximum health practices.
· Utilize community agencies and alternative resources to assist patients and their families in resolving unmet health care needs, financial and environmental difficulties.
· Report pertinent observations to case manager/supervisor and attending physician.
· Participate in the team process through cooperative efforts with team members in development of the patient care plan and team conferences and provide comprehensive, individualized care to assigned patients.
· Demonstrates effective communication skills with all team members, patients and families.
· Maintains clinical record information in accordance with established standards and within established time frames.
· Prepare schedule daily.
· Demonstrate cost-effective practices.
· Participate in the review process and the agency performance improvement program.
· Participate in orientation, in-service and staff meetings as scheduled.
· Secures patient confidentiality by protecting patient information on and off duty.
· Displays flexibility in accepting assignment changes and readily accepts assignment and patient care responsibilities which may include participation in weekend coverage and call rotation.
· Develop professional and clinical skills through attendance at staff development meetings, pertinent educational activities and professional affiliations.
· Complete related tasks as assigned by the Supervisor, Manager or Director.
· Maintains current driver’s license, automobile insurance and BLS.
Qualifications
EDUCATION AND EXPERIENCE REQUIRED:
· MSW Degree from a school of Social Work accredited by the Council on Social Work Education.
· Minimum of (1) year of social work experience in a health care setting
EDUCATION AND EXPERIENCE PREFERRED:
· One (1) year of Home Health or Hospice Social Work Experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
· Current BCLS
· Valid Drivers License
· Valid Care Insurance
· LSW License in Illinois
