At Kaniksu Community Health, the RN/LPN Care Manager – Obstetrics is not just responsible for providing excellent customer service. They are committed to providing an excellent patient experience while performing essential clinical or administrative work in a fast-paced, productive environment. YOU ARE an essential team member charged with assisting patients in managing their care at Kaniksu Community Health.
Under the direction of the KCH Director of Nursing, KCH Charge Nurse and Medical Staff Providers and as a member of the Kaniksu Community Health Patient Centered Medical Home (PCMH) Team; a typical day as the RN/LPN Care Manager - Obstetrics might include:
Oversee clinical care of other personnel including LPN's, CNA's and MA's
Provides case management by being a patient advocate, resource and contact person for patients
Provides basic education information to patients and families, focusing on self-care appropriate to their age and identified learning considerations and needs
Performs assessment, data collection and documents subjective data and objective data
Reviews basic health history information for patients; focusing on environment, self-care and clinical discharge planning
Telephone triage with patients regarding medication, treatment, test results, and providing guidance during emergent and/or urgent situations
Responsible for accurate and timely communication of lab results to patients and order and instruct patient(s) with medication and/or treatment plans, to carry out provider orders
Responsible for ongoing admission to the KCH CM program, including monitoring, follow-up, and discharge planning, as appropriate
Orients new clients to the perinatal programs and schedules appointments as necessary. Encouraged participation in support services offered
Assists in the training and evaluation of perinatal staff as appropriate
Completes the initial comprehensive support service visit per protocol for nutrition, health education and psychosocial concerns, begins care plan by identifying problems and strengths and establishing goals with clients and completes re-assessments each trimester of pregnancy including a postpartum assessment
Conducts educational classes regarding childbirth and breastfeeding
Coordinates care of perinatal patients, ensures that medical appointments are scheduled, ensures that supportive service and classes are offered and/or scheduled, ensure the documentation of all appointments made, missed, canceled, and re-scheduled (including follow-up) with those that are missed
Refers patients for follow-up appointments if needed for nutrition, psychosocial, medical and health education risk factors as described in protocols
Acts as a liaison between the client and team (medical provider, nutritionist, and counselor)
Collects, maintains, and participates in the development of patient education material
Participates in quality assurance evaluation meetings and participates in the QA review process for all clinics offering OB services
Assists the Medical Director and/or Director of Nursing with writing necessary protocols and policies to comply with Maternal Child Health (MCH) requirements and with the design and implementation of necessary forms, such as patient care plan, premature labor signs, patient financial agreement, prenatal and postnatal teaching, etc.
Conducts outreach and public relations within the corporation, as well as, with local community providers and agencies
Ensures that all necessary reports are completed in a timely manner: monthly report of terminated patients, monthly report regarding the progress of the program, etc.
Responsible for perinatal-related communications sent within the corporation as well as outside providers/agencies
Responsible for being informed of current perinatal legislation and ensuring all patient care is within professional practice standards, policies and procedures of KCH and the State of Idaho scope of practice
To land this gig, you need to have some experience helping people. It does not have to be traditional customer service experience. Just make sure we can see where you picked up your ability to help others in your cover letter or resume.
Minimum Associates Degree in Nursing; BA in Nursing preferred. Current license to practice as a RN or LPN in the State of Idaho
Minimum 1-3 years' experience in ambulatory care. Experience in a community health center is preferred by not required
Capable of independent nursing assessment decisions and nursing management in a primary care setting
Able to provide health care in a sensitive, non-judgmental manner to persons of all economic levels and cultural backgrounds
Demonstrate accuracy, proficiency and efficiency with computer skills for accurate data entry; effective written and oral communication
Consistent demonstration of team approach to patient care
An awareness of community services and a resourcefulness to obtain services for patients
Experience and/or training in mental health and substance use education and referrals
