Position Overview
The Division of Rehabilitation Services (DRS) is the states leaddivision serving people with disabilities committed to diversity,equity, inclusion, accessibility, and racial justice. The mission of theDivision is to assist individuals with disabilities in achieving theirgoals of employment, education, and independent living. The Division ofRehabilitation Services is seeking to hire a self-motivated anddetail-oriented individual with good communication skills. This positioncoordinates customer case records and documents in a field officepertaining to the service delivery activities to customers in the Bureauof Customer and Community Field Services/Home Services Program Thisposition will receive formal and informal training not to exceed twelvemonths to learn the rules, regulations, and policies of the HomeServices Program to ultimately manage a caseload providing services toindividuals with disabilities to remain independent in their home andcommunity.
Job Responsibilities
1. In the Division of Rehabilitation Services (DRS), Bureau of Customerand Community Field Services Home Services Program, receives on-the-jobtraining not to exceed twelve months to learn the rules, federal andstate regulations, policies, and procedures for provision of services toindividuals under the Persons with Disabilities (PWD) Waiver, BrainInjury (BI) Waiver and/or Acquired Immune Deficiency Syndrome (AIDS)Waiver at risk of nursing home placement.
2. Studies and becomes proficient in utilizing the Division\'s casemanagement system.
3. Attends and participates in staff meetings, conferences, workshops,and in-service training related to the discipline of rehabilitationcounseling and the home services program.
4. Performs other duties as assigned or required which are reasonablywithin the scope of the duties enumerated above

Conditions of Employment
1. Requires ability to travel to individuals home to interviewapplicants to determine initial eligibility and to monitor and revaluateservices for DRS Home Services Program.
2. Requires the ability to physically access and visually assess thehome environment for suitability and planning of services.
