Registered Nurse, Service Integration Coordinator InCK GrantRequisition ID2023-47437CategoryNursingOverviewPart-time, Hybrid role , 24 hr/weekThe Registered Nurse, Service Integration Coordinator works on a multi-disciplinary team that is responsible for assisting in the development and implementation of the Integrated Care Management Model and Care Planning Team Model as part of the Integrated Care for Kids grant award. A primarily telephonic role, the Service Integration Coordinator ensures quality patient care in the appropriate setting by assessing key needs, addressing local and systemic issues interfering with care. The service integration coordinator will interpret the top two service integration levels assigned to all Medicaid eligible children (0-20) living in Licking and Muskingum counties. Assessment information will be gathered and evaluated to confirm the most appropriate level of care coordination intervention based on the individual patient's needs. The Service Integration Coordinator will refer patients to services and resources in the community.The goals of the seven-year co-operative agreement between the Ohio Department of Medicaid, Nationwide Children's Hospital are to:1. Improve child health;2. Reduce substance use disorders and opioid use;3. Reduce avoidable inpatient stays and out of home placements;4. Assesses the child's/family's needs for case management oversight/intervention.
