Our Syracuse location is comprised of 2 buildings; the Cunningham building, a 13-floor, 487-bed long-term care skilled nursing facility, and Fahey Rehab, a 96-bed short-term residential rehabilitation program - the largest in Central New York. Our Fahey building offers specialized services including Palliative Care and Restorative Care. 　　Staff RN – Telemetry Step-Down Unit

　　Full Time, Part Time and Per Diem Available - 12-hour shifts

　　This completely renovated, 25-bed unit is designed to help those recovering from major cardiac surgery and diagnoses requiring hemodynamics and tele-monitoring, to rehab more comfortably in a more home-like setting. Every patient admitted to the unit is outfitted with a portable WIFI/cellular cardiac monitor which registers real-time data 24 hours a day, including when patients are in therapy. The information is transmitted to a central nursing station where our team has immediate access to any change in a patient’s status or condition.

　　Facility RN Supervisor – Long Term Care or Short Term Acute/Rehab

　　Full Time, Part Time and Per Diem Available

　　Direct the day to day functions of the nursing activities in the facility in accordance with current rules, regulations and guidelines that govern the long-term care facility. Responsible for making rounds on all to assist nursing staff in resolving issues/concerns. Ensure there is a sufficient number of staff members and that the needs of the residents are adequately met. Communicate with staff, physicians, and patients point of contact when there are changes of the patient’s condition or if immediate medical care is necessary. Review and evaluate your shifts work force. Monitor your shift’s personnel to assure that they are following established safety regulations.

　　Unit Charge Nurse, RN – 1 Short Term Acute/Rehab Unit

　　Full time, Part Time and Per Diem Available

　　Provide direct nursing care to short-term care of residents for medication administration and treatment for medical conditions/issues, including IV therapy, PICC, respiratory and wound care. Assess resident needs and initiates development of individualized resident care plans. Monitor residents for change in condition and work with physicians and the facility interdisciplinary team to implement physician orders and needed interventions. Supervises and evaluates direct resident care provided within assigned unit and initiates corrective action as necessary. Assists in the admission, transfer and discharge of residents as necessary.

　　Unit Clinical Coordinator, RN – Long Term Care or Short Term Acute/Rehab

　　Full Time Days - Monday through Friday

　　In collaboration with Unit Nurse Manager, the Clinical Coordinator performs initial assessments and re-assessments of conditions and responses to treatment as well as any other clinical findings. Develops and implements plans to meet patients’ needs, including pain management, wound/skin care and infection prevention. Educates residents/patients and regarding treatment measures. Promotes continuity of care through developing an individualized interdisciplinary plan of care in collaboration with the resident/patient, family and health care team.

　　Unit Nurse Manager, RN - Long Term Care or Short Term Acute/Rehab

　　Full Time Days - Monday through Friday

　　Oversee nursing operations and supervisory duties for entire unit. Provide direction to Clinical Coordinator/RN and Assistant Nurse Manager/LPN, to ensure quality care is being provided on all 3 shifts. Ensures complete and prompt reporting of incidents with follow-up as necessary to Administrator and Director of Nursing.

　　Unit Admissions/Discharge RN – Short Term Acute/Rehab

　　Split shift: 11:00 AM to 7:00 PM

　　Oversee all routine admissions and discharges. Complete the required assessments and documentation and ensure targeted discharge, education and home care needs are met.
