  Description

  As a Registered Nurse (RN) at Fenway Health you will provide compassionate, evidence-based, and efficient care to individuals, families, communities, and patient populations. The Registered Nurse’s care delivery is consistent with the Massachusetts Board of Registration in Nursing, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The RN demonstrates the professional role obligations of. Professional accountability enriches the RN's engagement as a leader in promoting an inter-professional culture of collaborative decision-making, innovation, life-long learning, and teamwork.

  At Fenway Health the RN will provide quality nursing care to patients consistent with the care philosophy of patient-centeredness, non-judgmental, harm reduction, behavioral health integration, and team-based care. The nurse serves in a clinical leadership role to the care team with primary responsibilities for disease management, care coordination, and advice/triage needs for the care team's panel of patients. The nurse ensures that the team functions smoothly and efficiently while keeping the patients' well-being paramount. The RN will set a positive example of nursing for all Fenway Health staff, residents, and visiting learners; and will always conduct their self in a respectful, courteous, and professional manner consistent with the Fenway Health Mission “to enhance the wellbeing of the LGBTQIA+ community and all people in our neighborhoods and beyond through access to the highest quality health care, education, research, and advocacy.”The schedule is based on clinic needs and may include covering evening and Saturday clinics on a rotating basis.

  Nursing is the protection, promotion, and optimization of health and abilities, prevention of illness and injury, facilitation of healing, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations.

  Principal Duties and Responsibilities

  Collaborates with the patient’s provider, specialist providers, and other members of the care team to improve patient outcomes through a patient-centered care plan incorporating patient engagement and education to promote health and prevent injury, illness, and disease exacerbation.

  Provides competent, compassionate nursing care based on the nursing process, clinical reasoning, objective scientific principles, and the patient’s subjective experiences.

  Conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately.

  Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the provider, patient, and other care team members.

  Influences appropriate utilization of healthcare resources by helping to coordinate patient care, encouraging patient involvement in disease management and wellness programs, and conducting follow-up care for acute care or psychiatric admissions, emergency department visits, specialist visits, and sub-acute / rehabilitation care stays.

  Implements direct and indirect nursing care consistent with evidence-based practices, hospital policies and procedures, scope and standards of practice, and Nursing’s Code of Ethics.

  Coordinates preventative services for the defined population including, but not limited to, education, preventative medications, vaccinations, testing, and referrals.

  Documents patient information accurately and in a timely manner in the EHR.

  Speaks up immediately about concerns regarding assigned responsibilities and available resources.

  Promotes a compassionate and therapeutic environment that is responsive to the unique patient and caregivers and extends to members of the care team.

  Assures a sensitivity to recognize, appreciate, and incorporate individual differences and preferences in the provision of culturally competent care for the patient and with the care team.

  Participates in daily operational activities necessary for a safe patient and team environment.

  Identifies complexities within the healthcare system, and Fenway Health, and participates in resolving barriers to effective, efficient, and fiscally responsible care delivery.

  Participates in planning, implementing, and monitoring nursing goals and objectives consistent with the Fenway Health Strategic Plan.

  Demonstrates the ability to work independently and as an effective healthcare team member.

  Evaluates own knowledge, as well as nursing peers, and nursing practice in relation to professional practice standards and evidence-based knowledge in consultation with peers and colleagues.

  Attends meetings and participates in committee functions and activities as nominated or assigned.

  Adheres to organizational and departmental policies and procedures in collaboration with other departments and in accordance with state, federal, and organizational guidelines.

  Demonstrates initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals.

  Exhibits a positive attitude with patients and establishes professional and respectful relationships with the internal and external healthcare team members, agencies, and healthcare facilities.

  Demonstrates ability to compile patient data and prepare outcome analysis.

  Ability to manage conflict, stress and multiple simultaneous work demands in an effective and professional manner.

  Ability to work well independently, while collaborating with other team members. Serves as a clinical resource person to staff.

  Ability and willingness to self-motivate, to prioritize and change processes to improve effectiveness and efficiency. Adapts to changing patient or organizational priorities.

  Ability to make independent decisions in accordance with established policies and procedures. Decisions and problem solving require a combination of analysis, evaluation and interpretive thinking.

  Knowledge of and appreciation for cultural diversity and low literacy issues in care provision.

  Computer literacy, including by not limited to, data entry, retrieval and report generation.

  Ability to work with patients/families of all ages and in a variety of settings, including office, facility and patients’ homes, presenting diverse physical conditions and social/cultural environments.

  Requirements

  Education: Associate degree in nursing

  Current Massachusetts Registered Nurse (RN) license degree required.

  Current CPR certification required

  Effective interpersonal and communication skills and ability to work harmoniously with diverse groups of individuals

  Experience working in an ethnically, culturally, and racially diverse environment

  Authentic interest in LGBTQIA+ and community health

  Proficient in the use of the computer and associated software, i.e., MS products

  Ability to work harmoniously and effectively with colleagues, patients, clients and vendors across the spectrum of diversity, including but not limited to race, ethnicity, color, gender identity, sexual orientation, age, socio-economic status, national origin and immigrant status, religious or spiritual identity, disability (physical, mental, emotional and developmental), veteran status, and/or limited English proficiency.

  Preferred Qualification

  Bachelor’s degree level or higher in nursing

  Two (2) years of experience in ambulatory, hospital, or public health nursing

  Experience with working within a complex healthcare system with multiple product lines

  Board Certification (ACRN, AMB-BC, etc)

  Working Conditions and Physical Requirements

  OSHA Category 1: involves exposure to blood, body fluids, and/or tissues

  Physical Requirements

  Frequently required: lifting and/or moving up to 10 lbs.

  Occasional required: lifting and/or moving up to 25 lbs.

  Noise Level: usually conversational

  Shift

  Required: ability to work a standard 37.5 hour work week and rarely, a work day may require a shift up to 12 hours
