Position Summary Women\'sIntensive HIV Nurse Case Manager Purpose In the role of Registered NurseCase Manager you will provide HIV nurse case management to youth (ages17-25) and women living with HIV. Works collaboratively withmulti-disciplinary care teams that may include clients and their supportsystems, practitioners, nurses, social workers, and other providers.Supports program requirements through data collection and timelydocumentation. Serves as a recognized expert in the care of peopleliving with HIV and as a consultant to health care providers withinFroedtert and the Medical College of Wisconsin and the community.Primary Functions Responsible for providing intensive nurse casemanagement of referred youth (ages 17-25) and women living with HIV.Complete intake, assessments and develop service plans. Provide carecoordination, HIV education and prevention. Complete home andcommunity-based visits working extensively to support medicationadherence, health literacy, and client engagement in medical care, whilemaintaining client confidentiality. Collaborate with a diverse group ofmultidisciplinary care team members (physicians, nurses, social workers,community providers, etc.), to proactively plan and implement care andservice plan goals. Monitor changes and respond to follow-up needs ofclients, follow through on identified referrals in a timely manner, andcommunicate changes and needs with all members of a client\'sinterdisciplinary care team. Manage and maintain program data forenrolled clients. Acquire necessary patient information from off-siteclinical venues and ensure accurate inclusion in the program database.Collaborate with the WI HIV Primary Care Support Network to ensureappropriate grant related data points are included in the Network datasystem. Seek out and develop client education materials on a wide rangeof topics (HIV prevention, disease progression, medication adherenceetc.), educate clients, their support systems, and members of themultidisciplinary team on treatment options for youth (ages 17-25) andwomen living with HIV.
