Description:
Endorses, interprets and enforces the objectives and procedures of facility and the Nursing Department.
Plans, directs, coordinates, delegates and evaluates the activities of all nursing personnel within assigned area.
Conducts daily rounds.
Reviews resident care plans for appropriate goals, problems, interventions, and revises as needed, based on identified nursing needs.
Ensures the continuity of care by informing supervisors and staff of unit status and related matters through written reports and verbal communication.
Conducts daily conferences with unit staff to review specific patient care, and to communicate and interpret recent policy and procedure changes.
Extends courtesy to all unit visitors and assists them to contribute to the emotional needs of all patients.
Training:
Week 1: M-W 8:30am-4:30pm ALL classroom based. Learning procedures, protocols, and EMR
Week 2 (which could the Thurs/Fri of 1st week into the weekend): Buddied up on 1st shift to learn floor rules and orient to the specific floors itself
Skills:
long-term care, nursing home, Care management, Managed care, Patient care, Rn license, Case management, geriatrics, Triage, EMR experience
Additional Skills & Qualifications:
Rehabilitation experience
experience supervising support staff
experience working with elderly population
Schedule:
We will accept only agency staff who can commit to a minimum of 3 days per week with an every other weekend schedule. Weekends are defined as Saturday and Sunday. Exceptions will be considered on a case by case basis .
• Night - 10pm-6am (2 openings)
• Day - 6am-2pm (6 openings)
Free parking onsite. There is no subway nearby. They would need to take the Q train to the B 49 or B 1 bus.
