　　Responsible for providing care to acute and chronic psychiatric andmental health patients including assessing, diagnosing, planning, andtreating conditions.

　　Essential Functions, Duties, and Responsibilities

　　Reports to Director of Primary Care for administrative functions.

　　Reports to Primary Care Medical Director for clinical operations.

　　Collaborates with Oversight Physician for clinical consults.

　　Provides direct and/or indirect care to a specialized caseload ofpatients with psychiatric needs.

　　Assesses, plans, implements, and evaluates patient care. Diagnoses andtreats both acute and chronic psychiatric and mental health issues anddevelops a plan based on evidence-based care.

　　Provides appropriate patient education regarding diagnoses andmedications.

　　Documents activities in the patient medical record.

　　Records level of care using CPT and HCFA evaluation/managementguidelines.

　　Collaborates with the interdisciplinary primary care and communityhealth team on shared patients.

　　Assumes responsibility for CME.

　　Demonstrates professional commitment through membership in specialtyorganizations.

　　Performs tasks and participates in methods of quality assurance asrequested.

　　Participates in the development, implementation, and review ofprocedures and policies and annual program review as requested.

　　Arranges for or refers patients to needed services.

　　Assures patient health records are maintained with concise, legible, andpertinent documentation.

　　Promotes positive image of the practice.

　　Adheres to professional ethical standards.

　　Maintains professional appearance.

　　Adheres to the Standards of Behavior.
