Position Summary: The primary objective ofthe Physical Therapist is to plan, organize, develop and implement aprogressive therapy program geared to improvement of resident\'sphysical functions, psychological, social and economic well-being.Follow facility guidelines, objectives and policies to maintainstandards of treatment consistent with legal and accreditingrequirements. ASSESSMENT Upon receiving PT orders, review client\'shistory and completes thorough assessment, including precautions. Uponreceiving PT screening request, review client\'s current situation andscreens individual for intervention. Communicate assessment/ screeningresults to client and appropriate family and/or staff. Complete requiredassessment/screening documentation. PLANNING Identify and prioritizeneeds based on assessment and establish appropriate plan with goalsCommunicate treatment plan and goals to client and family Collaboratewith health care team members to begin planning for appropriate andtimely discharge or transfer. IMPLEMENTATION Demonstrate ability toprioritize treatment procedures according to patient\'s needs and MDrequests. Implement interventions according to plan of care. Communicatepurpose of treatment procedures to client/family according to client\'slevel of understanding and modify plan accordingly. Communicatetreatment plan/appropriate interventions with assistants, initially andongoing Communicate ongoing treatment issues/plan to interdisciplinaryhealth care team members. Complete required documentation regardingdaily intervention. EVALUATION Review and evaluate effectiveness ofinterventions toward achieving patient outcomes Document appropriatereassessment and changes in plan of care. Communicate significantchanges in intervention to client /family and health care team members.Complete required documentation for discharge.
