The Phlebotomist draws blood from patients or donors of allages in hospitals, clinics, blood banks, or similar facilities for analysis orother medical purposes.

　　Essential Duties and Responsibilities:

　　Assembles equipment, such as tourniquets, needles, anddisposable containers for sharps, blood collection devices, gauze, cotton, andalcohol on work tray, according to requirements for specified tests orprocedures

　　Verifies identity of patient or donor

　　Converses with patient or donor and explains procedure

　　Applies tourniquet to arm, locates accessible vein, swabspuncture area with antiseptic, and inserts needle into vein to draw blood intocollection tube or bag

　　Knowledgeable in selection of appropriate collectiondevices, tubes and gauges

　　Withdraws needle, applies treatment to puncture site, andlabels and stores blood container for subsequent processing

　　Other duties as assigned

　　Minimum Requirements:

　　High School diploma or its equivalent

　　Successful completion of Phlebotomist training withsubsequent certification (if applicable) as a Phlebotomist

　　Minimum of one year relevant experience preferred

　　Complies with all relevant professional standards ofpractice

　　Participation and completion of Maxim's Competency programwhen applicable

　　Current CPR if applicable

　　TB Questionnaire, PPD or chest x-ray if applicable

　　Current Health certificate (per contract or stateregulation)

　　Must meet all federal, state and local requirements

　　Successful completion of new hire training as applicable tojob site

　　Understand patient confidentiality and HIPAArequirements

　　Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

　　Computer proficiency required

　　Must be at least 18 years of age
