Schedule Details:Eligible for a \$3,000 sign on bonus! 10 hourshifts, rotates every 3rd weekend - can be scheduled to work at MUK,OMH, or WMH What You Will Do: Assists pharmacists, physicians andnurseswith medication reconciliation processes to ensure that anydifferences between medications taken before admission and medicationsordered upon admission for a patient are intentional and void ofnegative patient incident. Assists with reconciliation process atdischarge in coordination with pharmacist and case manager.Offersdischarge prescription service through ProHealth Retail Pharmacy.Collects appropriate insurance and demographic data for prescriptionprocessing.
