This includes appropriateness of dose, route, interval, schedule and duration. Corrects significant drug-drug, drug-disease, or drug-food interactions. Consults with licensed independent practitioners about medication use issues and documents interventions in medication therapy. Prepares and checks medication and answers drug information questions.Responsibilities:Performs basic clinical pharmacy services including drug therapy monitoring and pharmacokinetics.Reviews and verifies investigational medication orders.Evaluates investigational drug therapy orders for appropriateness of dose, route, interval, schedule and duration.Corrects significant drug-drug, drug-disease, or drug-food interactions.Consults with licensed independent practitioners and documents interventions in medication therapy.Investigational Medication preparation and checking, including oral, and IV investigational medications.Controlled Substance dispensing and tracking.Provides drug information. Prepares instructions for staff pharmacists to use when dispensing investigational medications.Assists with revision or implementation of new policies or standard operating procedures.Assists with the evaluation, onboarding, and validation of investigational drug study protocols for the hospital, as part of a multidisciplinary team.Assists nurses and physicians with investigational medication use issues.Ensures compliance with all hospital, governmental and sponsor regulations for investigational medications.Manages and tracks records of ordering, shipping, receipt and inventory. Maintains records to meet sponsor, FDA and legal requirements.Coordinates pharmacy sponsor required audits and site visits.Assists with the resolution of investigational medication study deviations.Performs medication reconciliation.Validates investigational medication build in software programs needed to dispense investigational medications.Other duties as required.Technical ExpertiseExperience in pediatrics/hospital pharmacy is preferred, but not required.Proficiency in MS Office [Outlook, Excel, Word] or similar software is preferred.Proficiency in Epic Willow Pharmacy software is preferred.Proficiency in Vestigo investigational drug software is preferred.Education and ExperienceEducation: Bachelor of Science in Pharmacy or Pharm D Degree is required.Licensure: Current License by the Ohio
