Job Overview
The Pharmacy Medication Transition Technician assists inpatient and outpatientpharmacists with medication history, reconciliation, to ensure that anydifferences between medications taken before admission and medications orderedupon admission for a patient are intentional and not accidental. Works topromote an efficient discharge prescription process in collaboration with theoutpatient pharmacy.

Department: Outpatient PharmacyHours per week: 40, eligible for benefitsShift: Varies (day/evening), 4 -10-hour long shifts. Rotatingweekends, no holidays at this time.
Qualifications
MINIMUM QUALIFICATIONSEducation: High School Diploma or EquivalentExperience: Two (2) years of Pharmacy Technician Experience. MedicationReconciliation Technician or Meds to Beds experience is preferred.Equivalency:Completion of pharmacy technician program may substitute for one (1) yearof the experience requirement. ORCompletion of pharmacy technician program plus one (1) year of experienceperforming medication prior authorizations may substitute for two (2)years of pharmacy technician experience.Certification(s):State of Colorado Pharmacy Technician (PHAT) CertificationCertificate through a nationally recognized certification board or bodyapproved by the National Association of Boards of Pharmacy (NABP) such asthe Pharmacy Technician Certification Board (PTCB) or National HealthcareAssociation (NHA)Medication History Certificate within 12 months of hire
