Hourly Pay Rate: $20-$21.25
A Personal Development Mentor guides a person towards identifying their strengths, uncovering authentic talents and applying their unique abilities in pursuit of their goals. The Personal Development Mentor helps to bridge the gap between where a person is and where they want to be by assisting with problem solving, decisions making and goal attainment in daily life. This can be achieved by improving a physical skill, overcoming self-limiting beliefs and behavior, learning new behavior for personal growth and helping to move forward from a person’s present situation towards their overall goals.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
Promotes an environment in which individuals’ rights are upheld and individuals thrive. Ensures regulatory and agency compliance requirements are met so individuals are cared for in a human, programmatic and comfortable manner.
Directs and coordinates provision of services and integrated, active programming under direction consistent with all applicable regulations and in conjunction with other agency operations and objectives including strategic planning and implementation of programmatic goals.
Ensures efficient, confidential and comprehensive record keeping as mandated for individual and staff related activities/incidents.
Identifies and takes the necessary action to improve quality in all areas of responsibility and effect change in a timely manner.
Maintains commitment to provide services that are responsive to the changing needs as defined by our individuals.
Responsible to assess an individual and their current situation by meeting and gathering information from the person themselves along with members of the support team. Responsible for compiling the information and working with the individual to build person centered goals.
Defines the success of the goals, report status monthly and collaborate with support team members to work towards the goals until completion.
Provides direct one to one coaching and mentoring to the individual to develop sustainable connections to their community based on their overall goals.
Responsible for follow-up and periodic check-ins as per a discharge plan agreed upon by the individual and their team members
Ensures compliance to applicable Federal/State/Local and other governing entities’ rules, regulations and law.
Complies with all agency policies and procedures.
Other duties as assigned.
MINIMUM QUALIFICATIONS:
Associate’s degree and 3 years of related experience, or an equivalent combination of education and experience.
Valid Driver’s License that meets agency policy.
Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
Physical agility and ability to react to emergency situations
