GENERAL PURPOSE

　　Performs evaluations and develops effective patient treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing outpatient and home health care. Services may be performed in therapy clinic or individual patient residence.

　　Position: Occupational Therapist - OTR

　　Hours: We will consider Consistent PRN, Part and Full-time applicants

　　Flexibility - no weekends or evenings, Monday thru Friday

　　Setting: Assisted Living Community

　　Location: Waters at Eden Prairie, Eden Prairie, MN

　　QUALIFICATIONS

　　Graduate from Occupational Therapy program

　　Current license as Occupational Therapist or ability to obtain in the state of practice

　　Previous experience in outpatient, preferred

　　Must meet Clinical Competency requirements

　　Current CPR certification, if practicing in home health
