GENERAL DESCRIPTIONThis position is responsible for oversight and continued development of the mobile unit healthcare program based out of the PORCH, as well as liaise between ATS nursing staff and leadership, if applicable. This is an administrative/leadership role under the direct supervision of the Executive Director of Health Services/Medical Director.
REQUIREMENTS
Bachelor's Degree in Nursing OR Bachelor's Degree in Social Work or Human Services
If BSN, Graduate of an accredited school of nursing
If BSN, currently licensed as a registered nurse in the State of Michigan.
Preferred 3-5 years of administrative experience.
CPR/First Aid

Valid Driver's license required
ESSENTIAL RESPONSIBILITIES
Oversight of the mobile unit operations and processes as it pertains to provision of medical and behavioral health services.Provide supervision, or direct care, as it pertains to community outreach operations in Grand Traverse and surrounding countiesCollaborate with other mobile outreach team members, including Chief Program Officer, case managers, peer support specialists, etcMay include general oversight of all nursing staff; provide direction, supervision, and consultation for the work of agency-wide nursing staff
SPECIFIC DUTIES
Identify workflow improvement opportunities and provide recommendations to Executive Director of Health Services/Medical Director.```{=html}
```- Develop and coordinate a community triage process, based on acuity of patient need- Travel with PORCH/Outreach Team to provide on-site ground management, training, and oversite of medical services

```- Provide initial on-site patient assessments, evaluate for need for medical or behavioral health treatment, consult with APP/Physician on-call- Responsible as a liaison between Outpatient, PORCH, Residential, and Health Clinic staff to ensure continuity of care

```- Communicate with other community-based agencies to provide full wrap-around support for clients- Perform and interpret Point-of-care testing such as Urine Drug Screens, Pregnancy test, PBT and PBT, when necessary- Understands the rationale for the use of medications and treatments and correctly administers as needed, and if applicable- Responsible for planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of healthcare team.- Working closely with the Chief Quality Officer And Chief Program Officer to ensure delivery of services meet community/contractual requirements- Coordinate new intakes with Access Dept; streamlined registration process, health insurance validation- Mange data collection and reporting for grants as they relate to the Mobile/Rural Health Program- Responsible for developing and maintaining positive, professional, and collaborative relationships with clients and ensuring customer satisfaction- Assist Executive Director of Health Services/Medical Director with hiring, disciplinary action, terminations, and the development of performance reviews.
