　　Overview:

　　The MDS Nurse is accountable for the federally mandated assessments. The incumbent will accomplish these tasks by coordinating and overseeing the interdisciplinary assessments and the care planning process.

　　Responsibilities:

　　Gather information on facility residents for assessments.

　　Work within the time frames set by Federal and State Regulations for all MDS sections.

　　Assess charts and assist with communicate with IDT to create applicable care plans for residents

　　Prepare and track Medicare certifications and recertifications.

　　Qualifications:

　　experiencve in LTC

　　Be knowledgeable to MDS 3.0, CAA’s, Medicare Guidelines, and Care Planning.

　　Excellent written and oral communication, including English usage, grammar, and punctuation.

　　Ability to work independently and work under pressure with time constraints
