  Hours: 4 days a week 8:30am-4:30pm except Fridays are 7:30am-3:30pm

  Summary:

  Under direction and guidance of the physician and Director of Neighborhood Health Center, the Neighborhood Health Center LPN is assigned to the Hospital's Primary Care Clinic and accepts the responsibility for the delivery of patient care through the process of assessment, planning, implementation and evaluation as appropriate to the ages of the patients served.

  The Neighborhood Health Center LPN works in close cooperation with, and under the direction and guidance of, the physicians assigned to the Primary Care Clinic. The employee maintains established departmental policies and procedures, objectives, quality assurance programs, safety, environmental and infection control standards. Maintains confidentiality of all clinical and staffing information. The employee strives to incorporate the philosophy and mission of the hospital and to comply with the Patient Bill of Rights.

  Mount St. Mary's Hospital consists of multiple locations and associate may be assigned as needed.

  Responsibilities:

  EDUCATION

  Graduate from accredited school of Practical Nursing required

  LPN Licensure in New York State

  Basic Life Support Certification (must possess BLS within sixty (60) days of job offer

  HIV counseling certification preferred

  EXPERIENCE

  Minimum of either: One (1) year of recent pediatric clinic or pediatric physician's office nursing experience and primary care clinic or physician's office experience as determined by the Hospital; or one (1) year hands-on acute Medical/Surgical/Pediatric hospital experience as determined by the hospital.

  Extensive knowledge of third party resource utilization and referral requirements required

  Two (2) years adult medicine physician office/clinic experience preferred

  Compile statistics

  Compose letters/memorandums

  Input data into computer programs

  Maintain filing systems, logs, patient charts

  Proof-read documents

  Research information

  Schedule appointments

  Diagnostic equipment

  Fax

  Personal computer

  Photocopier

  Excellent communication skills, including both oral and written

  WORKING CONDITIONS

  Exposed to infection from disease-bearing specimens; hazards of handling diseased organs and tissues; housekeeping cleaning agents/chemicals; hazardous anesthetic agents, body fluids and wastes; unpleasant elements (accidents, injuries and illness)

  May be exposed to toxic chemicals (i.e. photocopier)

  Regularly exposed to the risk of bloodborne diseases

  Subject to many interruptions

  Occasional exposure to unpleasant patient or unit elements

  Contact with patients under wide variety of circumstances

  Handles emergency or crisis situations (potential patient violence); absentee replacement on short notice

  May perform emergency care

  Occasionally subjected to irregular hours

  Occasional pressure due to multiple calls and inquiries

  Flexibility in scheduled working hours/days to meet needs of unit

  Days and hours may change to meet the needs of the position

  Required Protective Equipment

  Eye/face protection may be required

  Gowns may be required

  Hair covering may be required

  Lab coat or apron may be required

  Mask may be required

  Non-Sterile/sterile medical gloves may be required

  Utility gloves may be required

  Hazards: conditions where there is danger to life, body, and/or health
