　　Qualifications:

　　Current LPN licensure by the Kansas State Board of Nursing.A minimum of one year of nursing experience in eitherpsychiatric/mental health nursing or in physician office practicepreferred.Must demonstrate competency in the use of technology includingcomputers, various software applications, printers, scanners,telephones, etc.Must also demonstrate a commitment to continuous improvement, toinclude understanding and application of technology (hardware,software, equipment and processes).Summary Description:

　　Assists and work with CCBHC Integrated Care Nurse coordinating careand follow-up on quality measures as indicated.Coordinates education on: Hypertension, Diabetes Mellitus,Weight/BMI, Cardiovascular, Immunizations, Tobacco cessation and SUDreferrals.Coordinates referrals to the CCBHC Integrated Care Nurse on: TobaccoCessation, SBIRT/SUD counseling and education.Coordinates lab orders as directed and per policy.Tracks and ensures that required assessments and screenings arecompleted for each client.Provides information about Center services and referral to Centerservices of individuals and groups needing Center services.Provides patient education in order to increase knowledge andimprove health and safety.Coordinates care with internal and external healthcare providers asneeded.
