　　Shift: Day, Evening- All candidates must be willing to work rotating weekends and rotating holidays

　　Position Requirements: 1 year of elder care or direct care experience is preferred but not required. NYS Nursing Licensure (LPN) required.

　　$500 SIGN ON BONUS AVAILABLE!

　　RPH is looking for LPN candidates who are passionate about caring for elders for our three communities. Responsibilities include but are not limited to; Medication monitoring and administration, acting as clinical lead to staff, case management, care planning and oversight, personal care assistance as needed, meal planning and enrichment recreation as needed.

　　Job Description:

　　Implement and evaluate the delivery of health services to elders.

　　Ensure progress notes are informative and descriptive of services provided and of the elders’ response to services.

　　Assist in providing daily care to Elders.

　　Function as an advocate for elders, coordinating and linking all aspects of their care with the case manager.

　　Ensure that Care Partners/Shahbaz are aware of each elder’s wellness plan and that these plans are used to provide daily care.

　　Develop and implement effective communication systems between staff; foster positive interpersonal relationships.
