　　POSITION SUMMARY:  

　　This position reports to the Integration and Care Management, Senior Manager and will function as an essential team member for Honor Community Health. The Community Health Worker (CHW) engage in collaboration with multiple team members of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of comorbid physical and behavioral health conditions.   

　　ESSENTIAL FUNCTIONS: 

　　Leadership and Training 

　　Assures that policies, processes, and procedures are consistent with CHW practice and align with programmatic goals by providing day-to-day guidance to clinical site CHWs. 

　　Monitors and reports for operational issues related to community health work and reports issues to Senior Manager. 

　　Onboarding and ongoing training for all clinical site CHWs, as directed by the Senior Manager. 

　　Organizes, arranges, and facilitates CHW trainings and in-services to improve practice across the organization.  

　　Positive and supportive work environment 

　　Independently performs the full range of duties assigned to the community health workers at Honor and encourages CHWs to do the same. 

　　Advanced skill in delivering culturally appropriate assistance to patients from diverse backgrounds. Works to create a positive and supportive work environment respectful of living and working in a multicultural society.  

　　Awareness of community health worker policies and procedures and can critically problem-solve within them when unique situations arise.  

　　May perform administrative support duties such as scheduling appointments, answering calls, or front desk work if the organization deems it necessary.  

　　Increase patient access to care and resources

　　Assess patient for needs related to housing, transportation, food, and other activities of daily living and connect the patients and/or caregivers to community and individual support services.  

　　Assists eligible patients with completion of applications for publicly sponsored health care insurance plans, utilizing the Health Insurance Marketplace and MI Bridges websites while educating clients on available benefits.  

　　Identifies community resources (i.e., social services, education, employment, etc.) and educates the patients/caregivers on how to utilize and access them as appropriate. 

　　Coordinates with patient to complete open referrals provided to patients/caregivers by their primary care clinician and documents outcomes within the Electronic Health Record.  

　　Provides education on health conditions and strategies to implement identified care plan goals including both clinical and non-clinical needs.  

　　Participate in organizational and programmatic quality improvement projects.  

　　Completes social determinants of health assessments as needed and documents results in the electronic health record.  

　　Documentation of work 

　　Documents patient interactions in the electronic health record and through other tracking methods as determined by the Senior Manager Integration and Care Management.  

　　Perform data collection and analytics relevant to role using population management software to create outreach lists per location.  

　　Meets regularly with the care team, acting as a liaison and advocate for the patient/caregiver, to discuss cases and formulate care plans and document created plans within the Electronic Health Record.  

　　Reviews transitions of care reports provided through the Health Information Exchange and outreaches to patient to arrange post-discharge follow-up.  

　　Community outreach 

　　Implements wellness and prevention initiatives per clinical staff (i.e., breast cancer prevention, immunizations, Medicare wellness visits, etc.). 

　　Facilitates health education groups as directed by supervisor.  

　　Acts as a liaison to the community by attending off-site events in an effort to assist community members and promote the services provided at Honor Community Health. 

　　Assists with increasing new patient and repeat care visits by promoting the center(s) within the community and connecting individuals to potential referral sources. 

　　Meets with patients one-to-one in the community, health centers, or other locations according to Honor Community Health policies and procedures.  

　　Communication and time management 

　　Maintains confidentiality and protects health information according to HIPAA standards.  

　　Effectively demonstrates superior interpersonal communication skills, developing and appropriate and professional rapport with patients, families, office staff, and external resources.  

　　Working independently, manages time by prioritizing workload appropriately. 

　　Performs other duties as assigned.  During a public health emergency, the employee may be required to perform duties similar to but not limited to those in the job description. 

　　What are we looking for?

　　A High School Diploma or equivalent is required 

　　Current MiCHWA Community Health Worker certification 

　　2+ years’ experience working in a clinical environment required or as a community health worker. 

　　Computer skills (Microsoft Word, Excel, Outlook, NextGen, CHAMPS, Azara)
