Intensive Tracking CaseWorker - Waukesha Job Code:2022-CSCA-027 Location:WaukeshaDepartment:Waukesha Adult & Juvenile Services FT/PT Status:Full Time JobResponsibilities: Caseworker- Intensive Tracking Position Summary:Provide intensive in-home monitoring and support to at risk juveniles,who are either at high risk for placement out of their homes orreturning to their homes from corrections or other institutionalplacements, with the goal of maintaining these juveniles successfully intheir homes/community. Essential Functions: Process assigned referrals,opening electronic and paper files. Attend initial staffing and followup case reviews. Maintain face-to-face contact with the juveniles intheir homes, at school, and in the community, as determined at theinitial staffing and follow up reviews. Provide youth with intensivesupport, mentoring, and monitoring to assist them with the specific\"areas of focus\" as determined at the initial staffing. Ensure youthfollow through with their court ordered supervision and Department ofHuman Service Treatment Planning. Provide 24-hour coverage, via cellphones, for crisis intervention and response to violations. Maintainregular, weekly minimum contact with the social worker to collaborate onthe youths\' behavior/progress, to report any concerns or violations oftheir court order as they occur. Attend any/all necessary Departmentstaffings or court hearings, as requested by the social worker. Maintainregular contact with the parents/guardian, as determined at the initialstaffing, to collaborate on the youth\'s behavior and to provide ongoingsupport to the caretakers. Maintain contact with the youth, schools, asdetermined necessary and appropriate at the initial staffing, to monitorthe juveniles\' compliance with attendance and behavior at school.Provide coverage for other IT Caseworkers\' caseloads when they areabsent from work, when necessary. Complete all necessary paperwork andparticipant reports per policy/procedure. Enter and maintain database asrequired by the program. Monitor youth on the electronic monitoring (EM)bracelet.Per EM policy manual, responsible for hook-ups/placement ofclients on the system; maintaining proper care and return of EMequipment; monitoring clients\' schedules and follow through with theconditions/rules of electronic monitoring; having regular contact withthe Monitoring Center/parents/social workers in response and follow upto violations; and completing the necessary paperwork and reports inregards to clients on the bracelet.
